Team Structure
The Word template provides a crisis leader and alternate plus functional teams. Record each lead and member’s name, designation, contact number and alternative contact. Assign real decision-making authority and substitutes before a disruption.

Completing and Maintaining the Form
Discuss interfaces between teams, activation triggers and reporting to management. Keep personal contact details in a restricted internal copy, review after personnel changes and rehearse the team’s communication.
