Report and Replacement Documents
For a still-valid physical card, apply personally within seven days. Bring a valid travel document, the institution’s current-student letter and a statutory declaration/original police report for loss. A recovered card reported lost must be surrendered within seven days. Replacements are digital.
Fees and Venue
First loss S$100; later losses S$300; damage S$60. Attend ICA Services Centre, Level 3, 2 Crawford Street during the service hours stated by ICA. A digital retrieval problem is different from losing an old physical card.
